Getting Started
The following steps will be important for my ability to provide you with the most accurate and timely care and advice.
- Obtain a copy of your breast biopsy pathology report and the breast imaging reports that led to your diagnosis. You can request copies of these documents by contacting the department where your breast imaging and/or biopsy was performed or, if the institution has electronic medical records, by asking that the report be released to your online medical record or chart. If you are having difficulty obtaining these documents, I can walk you through the process.
- Download, complete and save a copy of the following documents New Patient Health History form, Notice of Privacy Practices Form, Acknowledgement of Notice of Privacy Policy Form.
Survivorship Care
In order to provide you with the most optimal care, reviewing your cancer treatment records prior to your appointment will be needed. Your treating healthcare facility will require your written authorization to release your records to our office (By Your Side LLC).
